Position Overview
We are seeking compassionate and experienced Travel Licensed Practical Nurses (LPNs) to provide home health visits for patients in need of skilled nursing care. This role is ideal for LPNs who enjoy autonomy, one-on-one patient care, and the opportunity to travel to new communities.
Responsibilities
• Monitor patients’ vital signs and administer ordered medications and treatments.
• Educate patients and families on health maintenance, medications, and disease management.
• Collaborate with RNs, therapists, and coordinators to manage care.
• Accurately complete documentation according to agency and regulatory standards.
Qualifications
• Active LPN license in good standing (multi-state compact license preferred).
• Minimum 1 year of nursing experience (skilled nursing experience preferred).
• BLS certification required.
• Strong time management and communication skills.
• Ability to work independently and travel between patient homes.
• Reliable transportation required (mileage reimbursement provided).
